CCSP
WHAT IS CCSP?

The Community Care Service Program(CCSP) provides services to frail,
elderly and disabled individuals so they can remain in their own home or
community as an alternative to nursing facility placement.

Although it is a Medicaid Waiver Program, CCSP is not a type of
Medicaid.  In order to benefit under CCSP, the client must also receive on
or more services under CCSP.  If services are terminated, the client also
loses the Medicaid benefit.  Clients may incur a cost share (co-pay) for
CCSP services.

Nurse Management Services of Georgia, Inc. is a state licensed and approved for
funding by Medicaid for several waiver programs.  Our home care services works as
an extension of the physician's office, following the physician's orders to provide
patients the nursing care they need in the comfort of their own home.  We provide
quality care and services to seniors, the physically challenged, and those recovering
from surgery or illness.

within the four (4) regions serviced by Nurse Management Services of Georgia, Inc.,
please call us for more information on how to contract services or how to enroll your
client or relative to receive services from NMS.  We have a nurse manager who is
available 24 hours daily to answer your concerns and provide information.

WE PROVIDE:

Personal Care, Sitter Service, Companion Care, Bath Visits, Wound Care, Skilled
Nursing Care, Light Housekeeping, and Diabetic Care all in the comfort of your own
home.  All caregivers are screened, tested, credentials verified, and are submitted for
drug screening and background checks.
What is SOURCE?

Service Options Using Resources In Community Environments (SOURCE) was developed
specifically to address chronic physical illness in older and disabled recipients, behavioral
health diagnoses do not exclude a person from eligibility as long as one of the targeted
conditions exists.

Individuals eligible for SOURCE are:
a) Aged 65 and older, or under 65 and disabled
b) Receiving Medicaid under SSI (Supplemental Security Income, a federally administered
financial assistance program which provides payments and Medicaid benefits to low income
individuals who are aged, blind or disabled or Public Law categories*)
c) Qualifying as voluntary Members of the SOURCE Demonstration Project
d) Residing in a SOURCE Enhanced Case Management’s designated service area; and
e) With assistance from SOURCE and/or informal caregivers, capable of safely residing in the
community (with consideration for a recipient’s right to take calculated risks in how and where he
or she lives)  A person may not participate in more than one Medicaid waiver program at the same
time. Individuals may transfer from one waiver to another, contingent upon eligibility and available
funding.
