FORMS
TIME SHEETS: Time sheets are due in our office on Mondays by 12 noon. All time
sheets received after that time will be processed on the following pay period. Time
sheets can be mailed, dropped off, or faxed. Time sheets that are faxed must be mailed
to the office by the end of the current month. Not mailing original time sheets will delay
paychecks.
PERMANENT PLACEMENT: We provide permanent placement with our clients. When
you accept an assignment, this will be your permanent assignment, unless told otherwise,
that you will be scheduled to see on a regular basis. If you are sick or otherwise unable
to see your clients on any given day, you must follow the protocol for cancellation
outlined in your policy and procedure booklet. Another employee will fill-in for you until
you are able to return to work.
Cell Phone Use: All Cell Phones must be set to the vibrate setting while working with a
client. All calls must be kept to a 2 minute minimum and used only in cases of an
emergency. If, in an emergency, you must use the client’s telephone, consent must be
obtained first from the client and must be kept to a 2 minute minimum.
UNDER NO CIRCUMSTANCES MUST LONG DISTANCE CALLS BE MADE ON THE
CLIENTS TELEPHONE. IF YOU ARE WORKING OUT OF THE AREA AND YOU
NEED TO CONTACT THE OFFICE, PLEASE USE OUR TOLL-FREE NUMBER THAT
IS FOUND ON THE FRONT COVER OF THE BLUE FOLDER FOUND IN THE CLIENT’
S HOME. YOU WILL BE CHARGED FOR THE CALL IF YOU USE THE CLIENT’S
TELEPHONE FOR A LONG DISTANCE CALL
